• the URL or a clear description of where the material appears on our site;
• a statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent or the law;
• a statement, under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or authorized to act for them; and
• your physical or electronic signature.

Counter-notice

If material you posted (for example, a practitioner profile photo or bio) was removed and you believe that was a mistake, you may send a counter-notice to the same address with your contact details, a description of the material and where it appeared, a statement under penalty of perjury that you believe it was removed by mistake, and your signature.

Repeat infringers

We may remove content and suspend or close the access of anyone who repeatedly infringes others' rights.
